The jacks I received are the ones pictured. They where easy to handle. and worked great.

Fits any standard keystone jack. Plug and play. Works great and the sound is great. If you are setting up a home theater with wall outlets, this is what you need.

I bought a couple of these, and they worked perfectly. As a side note, I placed them into a decora face plate insert, which means there was less than the usual ammount of room for them (or any other keystone insert) to fit side by side. These are sized correctly, but those particular faceplates are under sized a bit. The good thing is, keystone inserts are made of plastic, and once you figure out where the crunch is happening... a little trimming with a utility knife goes a long way ;)
